Many women who have survived breast cancer describe ongoing memory problems after chemotherapy, often called chemobrain or brain fog. Yet the memory tests typically used in clinics, like word-pair or list-learning exercises, often fail to detect these difficulties. A new study published in Translational Neuroscience is the first to directly compare a standard memory test against a story-based test built around realistic, everyday scenarios.

Breast cancer is the most commonly diagnosed cancer among women worldwide, and chemotherapy has long been associated with cognitive changes. These can include trouble recalling personal events, difficulty following conversations, struggles switching between tasks, and slower response times, challenges not unique to breast cancer or to women. Survivors in the study reported needing more mental effort to complete complex real-life memory tasks, along with greater trouble recalling both specific events and general knowledge in daily life.

To investigate, researchers recruited female breast cancer survivors at least six months past treatment and matched them by age with a control group of women without a cancer history. Both groups completed two online verbal memory tests. The first, verbal paired associates, involved hearing word pairs, such as shoe-banana or book-fingers, then identifying which word was paired with each target. The second, Taler Stories, asked participants to recall details from short narratives describing everyday events, such as a family visiting grandchildren in New York.

Survivors performed about as well as the control group on the word-pair test, but recalled fewer details from the story-based test. The researchers concluded that standard clinical assessments can understate how much chemotherapy-related memory problems affect survivors' daily lives.

Study author Melanie Sekeres from the University of Ottawa explains: "One of the most exciting contributions of this work is that it provides objective scientific evidence supporting the memory difficulties commonly reported by breast cancer survivors experiencing brain fog, even when these impairments are not always detected by standard cognitive assessments."

The researchers still see value in standard memory tests for survivor care, but recommend pairing them with assessments that better reflect daily demands. While the study focused on women treated for breast cancer, broader use of such tools could help identify chemotherapy-related brain fog, clarify the long-term challenges survivors face, and inform interventions such as dietary changes, physical activity, and stress reduction.
