A study published in the American Journal of Psychiatry examines the neurobiological differences among individuals with schizophrenia, offering insights into the complex nature of this mental health condition. The research, led by Wolfgang Omlor from the University Hospital of Psychiatry Zurich, examined brain structures of patients with schizophrenia to identify areas of similarity and variability.
Schizophrenia manifests differently in each patient, with some experiencing primarily perceptual disturbances while others face cognitive impairments. Omlor notes, "In this sense, there is not one schizophrenia, but many, each with different neurobiological profiles." This variability underscores the need for precision medicine approaches in treatment.
The data was taken from the ENIGMA collaboration, a research project that combined imaging data from more than 6,000 people in 22 countries. The team examined various brain characteristics, including cortical thickness, surface area, folding patterns, and volume of deeper brain regions.

Key findings revealed both similarities and differences in brain structures among schizophrenia patients. Notably, the study found uniformity in brain folding in the mid-frontal area, suggesting a common developmental trait in schizophrenia. This pattern, largely established in early childhood, indicates less flexible brain development in areas responsible for linking thinking and feeling processes.
Conversely, other brain regions showed high variability, potentially reflecting the diverse symptom profiles observed in patients. Philipp Homan, professor at the University of Zurich and corresponding author, explains, "While uniform brain folding may indicate possible mechanisms of disease development, regions with high variability in brain structure may be relevant for the development of individualized treatment strategies."
