A comprehensive meta-analysis published in the British Journal of Sports Medicine reveals that cardiorespiratory fitness is a more reliable indicator of mortality risk than body mass index (BMI). The study, which analyzed data from 398,716 adults across 20 studies, challenges the conventional focus on weight loss as the primary strategy for improving health outcomes.
The research team, led by Siddhartha Angadi from the University of Virginia, found that fit individuals had similar mortality risks across all BMI categories. Conversely, unfit individuals showed a two- to three-fold increase in both all-cause and cardiovascular disease mortality risk, regardless of their BMI. Notably, obese but fit individuals had a lower mortality risk compared to normal-weight unfit individuals.
"Fitness, it turns out, is far more important than fatness when it comes to mortality risk," Angadi stated. He emphasized that exercise is not just about calorie expenditure but serves as excellent "medicine" to optimize overall health.

The study's findings suggest that improving cardiorespiratory fitness could be a more effective approach to reducing health risks than focusing solely on weight loss. This is particularly relevant given the challenges of maintaining weight loss and the potential health risks associated with yo-yo dieting.
Current U.S. guidelines recommend adults engage in at least 150 minutes of moderate-intensity or 75 minutes of vigorous physical activity per week. However, only about 20% of U.S. adults meet these guidelines. The researchers note that even modest increases in physical activity, such as brisk walking several times a week, can significantly reduce mortality risk for sedentary individuals.
