Beta-hydroxybutyrate (beta-HB, also called 3-hydroxybutyric acid) is the most abundant circulating ketone body, produced in the liver primarily from fatty acid oxidation and exported to peripheral tissues as an alternative energy substrate during fasting, ketogenic diets, or prolonged exercise. A beta-hydroxybutyrate assay typically measures this analyte through an enzymatic oxidation reaction using 3-hydroxybutyrate dehydrogenase, which converts beta-HB to acetoacetate while reducing the cofactor NAD+ to NADH. Colorimetric kits couple this NADH generation to a dye system read around 450 to 500 nm depending on the specific kit chemistry, while fluorometric kits instead couple NADH to a fluorometric developer that yields a highly fluorescent product, typically read around Ex/Em 530-540/585-595 nm.
